A Florida Mom's Lonely Fight for Her Disabled Son ? HCAFeNews

Tereza Pereira had cared for her woefully disabled son at home for most of his life. But she was in her 50s now, working two jobs to stay afloat, and state health administrators had repeatedly refused to pay for enough in-home nursing care to keep Bryan safe.

Pereira wanted her teenage son to live at a place called Baby House, a small group home for medically fragile children and young adults, with a long track record of treating children like Bryan as family. His care would have cost the state $300 per day there.

State health and disability administrators had a different plan: For $200 more each day, Bryan would live in a nursing home.

?I don?t want my son in this place,? Pereira wrote to disability administrators of the Florida Club Care nursing home in Miami Gardens. ?If something happened with my son, (if) he died,? she wrote, ?I will feel that this place killed? him.

Two years later, that is just how Pereira feels.

Disability administrators insisted that Bryan move to the nursing home. And there he died, a year later, on July 29, 2010.

?The best place for Bryan was with me,? Pereira said. ?I wanted my son to leave this world in peace ? not the way he passed away.?

Bryan Louzada was one of five medically complex children to die at Florida nursing homes in the last six months of 2010 ? and among 130 such children who have died in those homes since January 2006, records show. Though medically fragile children who live with their parents, or in a community setting, also die, state records show they die in far lower numbers.

State health administrators insist that the choices of parents like Pereira are the guiding force behind their decisions on where sick children live. But interviews and records show Pereira had fought for half Bryan?s life to find a homelike setting for him. And at every turn state health and disability chiefs steered him toward an institution or nursing home.

It is the dirty little secret of Florida?s health and social service system: Though institutional care can be dramatically more expensive than in-home care, state agencies push children toward institutions.

Here?s why: Medicaid, the state and federal insurance program for needy and disabled people, has become the insurer of last resort for virtually all children with catastrophic disabilities. Under federal law, a nursing home or facility bed is an entitlement, and that means Florida health administrators must provide such a bed to any family that asks. Sometimes-far-less-costly in-home nursing services are not an entitlement. Because they can, Florida lawmakers cap spending for such care, resulting in a waiting list of 25,000 for home- and community-based services.

Federal health polices ?lead to irrational outcomes,? said Jim DeBeaugrine, who was director of the state Agency for Persons with Disabilities under former Gov. Charlie Crist. ?People go into higher-cost facilities than what they need, and, quite frankly, what is best for them.

?It?s referred to as the ?institutional bias,? and that?s what the system has, because that?s where the dollars have to go. It?s nonsensical.?

The irrational outcome in Bryan?s case was that a sickly teen was forced to live in a nursing home that is considered one of the state?s worst ? Golden Glades, formerly known as Florida Club Care, is on the state ?nursing home watch list? of homes that did not meet even minimum standards of care during a recent inspection.

After a year of pleading, Pereira said disability administrators told her they would consider removing him from the nursing home in May 2010. Before they could, he died.

?I didn?t have a choice,? Pereira said. ?Now Bryan has passed away. What will I do with the rest of my life??

The U.S. Justice Department has sharply criticized the state, saying it is warehousing disabled children like Bryan in adult nursing homes, violating their rights in the process.

Liz Dudek, head of Florida?s Agency for Health Care Administration, has vigorously disputed that.

?The bottom line is simple: Florida cares about kids,? she wrote to the?Miami Herald.??We provide every medically necessary service and we are in compliance with the law.?

Officials with Golden Glades did not return phone calls seeking comment.

A second chance

Pereira, a psychologist in her native Brazil, was still grieving the AIDS-related death of her 5-year-old son, Bernardo, who had hemophilia and had been given an infected blood transfusion, when she learned she was pregnant with Bryan. It was as if she?d been given a second chance.

But when Pereira was 27 weeks into her term ? and on a trip to Miami ? she suddenly went into labor. The consequences were calamitous.

Bryan suffered a hemorrhage, and spinal fluid flooded his skull. He began to experience seizures. Before doctors could insert a shunt to drain the cerebral fluid, he contracted meningitis, a life-threatening infection.

In the nursery at Jackson Memorial, Bryan?s head swelled, his eyes bulged. Pereira went to a local Catholic church to pray when she was interrupted by a parishioner. ?A lady touched me. She said, ?talk to me.? ? The woman was a pediatrician at Miami Children?s Hospital, and Pereira transferred her son there.

?My son is dying,? she said. ?I have nothing to lose.?

At Miami Children?s, doctors inserted two shunts into Bryan?s underdeveloped brain and fought his infection. And though he slowly recovered, the list of his final diagnoses proved to be long and terrible: cerebral palsy, mental retardation, blindness, seizures, severe reflux, asthma and hemophilia.

Medical professionals told Pereira to send her son to an institution. Instead Pereira ? whose husband returned home to Brazil, leaving her with two small children ? took him home, and organized her small Kendall apartment into a makeshift hospital.

For the next 18 years, Pereira kept detailed records of Bryan?s health and his often-cruel journey through the state?s medical and social service system.

There were visits to the pediatrician, the hematologist, the neurologist, the gastroenterologist and the child development experts. Physical therapists stretched and relaxed Bryan?s legs, and occupational therapists worked to keep his fists from becoming permanently clenched. Speech therapists helped him suck and swallow.

Doctors charted every milestone, though they were few, and far between. At one year of age, Bryan had the cognition and the fine motor skills of a one month old.

Bryan remained with his mother until he was about 6.

Pereira says disability administrators refused to provide her with enough nursing hours to enable her to both work and be a caregiver. Hospital case notes quote a case worker as saying Pereira ?lacked the facilities at home to provide care for Bryan.?

Bryan?s older sister was a teenager, and was struggling over the caregiver demands on her life. ?She was crying. She said, ?I want a normal life. I can?t stand this anymore.? ?

That?s when the Department of Children & Families intervened, strongly recommending Bryan leave the house, Pereira said.

Pereira felt like the state was forcing her to choose between her two children.

?All the social workers and doctors pressed me to put my son in a group home, because my daughter was very depressed, without any attention,? Pereira wrote years later in an email to the state. ?But Bryan will continue to be my son, and I am his guardian.?

Pereira said she asked disability administrators to place her son in Baby House, a specialty group home run by United Cerebral Palsy, a national disability provider. Pereira had toured the home, and liked what she saw.

But Baby House is not an institution, and Medicaid administrators refused to pay. Bryan ended up in what is called an intermediate-care facility, an institution for people with severe disabilities.

Bryan spent most of the next decade bouncing between developmental disability institutions, nursing homes and Miami Children?s, which had become the hospital where Pereira felt her son was safest. In 2005, when Pereira had to move her son from one facility to another, disability administrators gave her a state handbook. ?You?re The Driver? it said, under a picture of a gas station. The message: Parents and guardians are decision makers over how and where loved ones receive care.

To Pereira, it seemed like a cruel joke. ?He was my son,? she said. ?I wanted to make the decisions for him.?

Pereira?s records document a litany of disappointments: At one institution in Miami, Bryan was repeatedly bitten by another child. At a nursing home in Plantation, his hemophilia was so poorly controlled, she said, that he developed raccoon eyes from internal bleeding.

Reordering her life

By the mid 2000s, Pereira had had enough. Bryan was at a nursing home in Broward for only a month when Pereira called police to report he?d been sorely neglected, police reports show. ?I was scared he would die,? she said. ?I was scared he would suffer.?

She removed him from the nursing home, and took him home ? again.

Photos from that time show how Pereira rearranged her world to take care of her then-16-year-old son. She moved all the furniture out of her cramped living room to accommodate Bryan?s hospital bed. White plastic shelves were moved in to store diapers, wipes, a dozen medications and a host of medical gadgets and supplies. An IV towered over the bed, along with an adjustable lamp to illuminate Bryan when his mom fed him and administered medications.

But the ping-ponging continued: Unable to care for Bryan with limited in-home nursing. Pereira agreed to send him again to a disability institution. ?What time am I going to take a shower? I am alone,? she said. ?If I had to go to the supermarket, who would take care of Bryan??

Bryan went to Baptist Hospital in Miami, then back to Miami Children?s. The hospital was eager for him to leave, but Pereira was not eager to place him in another nursing home. ?The hospital is making my life impossible,? she wrote to the state, adding social workers were insisting Bryan once again go to a nursing home.

Bleak choices

The state offered Pereira two choices: Bryan could return home with 10 hours each day of in-home nursing help ? an option a judge later called ?unacceptable as it would not allow the mother to work, and would not meet (Bryan?s) medical needs, as he requires an extensive amount of care just for feedings.?

Or he could go to a nursing home.

Pereira wanted a third choice. She had been to Baby House, in a small, nondescript ranch house off of Northeast 163rd Street in North Miami Beach. Baby House?s director, Carol Montiel, had visited Bryan. ?She said my son is a lovely, handsome boy,? Pereira wrote, and she would take very good care of him.

At Baby House, she wrote, staff would see him as a human being, ?not as a dollar.?

In March 2007, Pereira began her futile campaign to move her son to Baby House. She applied for a state program that provides money for noninstitutional care for disabled people, and was, instead, placed on a long waiting list.

Pereira wrote a longhand note to health administrators. ?I am alone in this country,? she said. ?I am not able to care (for) Bryan in home even with home care,? because the nursing hours weren?t sufficient, and because the nurses often failed to show up, or showed up late, even when they were scheduled.? It was, she wrote, ?a disaster.?

A March 24, 2007, email to the Department of Children & Families had this in the subject line: ?Please. I beg.?

?I want to beg, please, that my son be approved? for community-based care, the only funding pot the state could use to pay for Baby House. ?I don?t have anybody in this country. All my family is in Brazil,? she wrote

?If Bryan survived so far, it is because I have been a good mother caring for my son,? she added. ?I cannot permit Bryan to suffer any more.?

The waiting list

To get Bryan into Baby House, Pereira would first have to get him off the state?s disability wait list ? where he was languishing along with 25,000 other Floridians. The only way to get off that list, records show, would be for Bryan to become ?homeless,? or ?in danger of being neglected or abused.? Pereira was being punished for being a good mother.

In a letter dated July 23, 2008, a worker told Pereira the Agency for Persons with Disabilities planned to cut spending on those in community-based programs to accommodate a few clients stuck on the waiting list. The shift, which might have moved him up a few notches, was referred to as a ?cost efficiency.?

?They were only interested in dollars and cents, and I told them that,? said Carol Montiel, the Baby House director who had been speaking with Pereira for years, and was trying desperately to help.

Pereira wrote letters to then-U.S. Reps. Kendrick Meek and Debbie Wasserman Schultz, and Sen. Bill Nelson. She went to Washington for a family advocacy day and was pictured in the?Herald?alongside Rep. Ileana Ros-Lehtinen.

?Please remember,? she wrote to the state, ?that Bryan is a person with very involved medical and emotional needs, and not just another ?case.?

?I want to be with my heart full and happy that I did the best for him. I don?t want him to suffer anymore.?

Two of Pereira?s thick files document a desperate mother?s attempt to keep her son out of a nursing home ? in legal format: notices of continuance, subpoenas for deposition, proposed orders to dismiss, multiple notices of hearings, and a final order.

At a hearing in July 2007, an administrator with Florida?s Agency for Persons with Disabilities and a nurse with the state?s Agency for Health Care Administration testified that Bryan?s medical needs could be met in a nursing home. Pereira, who speaks fluent Portuguese but strained English, represented herself. The state, she said, ?hired a lawyer to go to court.?

A state administrative law judge ruled against her two months later. He offered Pereira a half-sentence as a consolation:

?That (Bryan) has survived to the age of 15 is a testament to his mother?s devotion, and the skilled medical care he has received since birth, primarily at Miami Children?s Hospital.?

Bryan was sent to the nursing home.

He remained there for a year, and his mother drove the length of Miami-Dade County as often as she could to see him.

In the middle of the night on July 29, 2010, Pereira says, she had a premonition: ?I called there to ask how he was. They told me he was fine; he was sleeping.?

At 5:30 a.m., Bryan was rushed to the Joe DiMaggio Children?s Hospital Emergency Room in Hollywood. Though she had instructed the nursing home to transport him whenever his heart rate exceeded 130, his heart had raced to 178, she said. ?Somebody should have noticed.?

At the hospital, Pereira found a doctor performing CPR. She yelled at him to stop. The compressions, she said, would cause him to bleed uncontrollably.

?I said, Bryan, Mommy is here. I will be with you.?

Bryan died at 11:39 a.m. Cause of death: respiratory failure. Pereira said she wanted an autopsy, wanted to know if the death could have been avoided, but doctors told her she?d have to pay $4,000 to get one.

Without an autopsy, a lawyer told her, she had no recourse against anyone.

?He was safe with me,? Pereira said.

Aside from the nursing home, Pereira said she holds state social service administrators responsible for Bryan?s death ? for forcing her to make a terrible choice.

?I want to know the reason why my son passed away,? she said.

Athens area talent hit it big in the country music industry in 2012 ...

Georgia has become a hotbed for music talent in the past few years, from Macon?s Jason Aldean, Leesburg?s Luke Bryan, to two members of Lady Antebellum, the Zac Brown Band, Sugarland and Billy Currington.

Even this year?s ?American Idol? winner, Phillip Phillips, is from Georgia.

?I don?t think it?s any rhyme or reason to it,? Oconee County native and country rapper Colt Ford said recently while shooting a music video outside Athens. ?It?s just, you know, things are popular right now and a lot of Georgia artists are doing good.

?But the one thing that we have in common is we don?t sound alike, which is really cool. You don?t get me and (Bubba Sparxxx) mixed up together. You?re not going to get Lady Antebellum and Zac Brown Band confused and you?re not going to get Brantley Gilbert and Luke Bryan confused. Jason Aldean sounds like he sounds and Sugarland. Thomas Rhett has his own sound. You?ve got little Lauren Alaina doing good.?

Several Athens area natives have been busy making a name for themselves in the music business this past year as well.

Monroe native Tyler Hubbard may not be a name many recognize at the moment, but country fans might recognize the name of his group: Florida/Georgia Line.

Florida/Georgia Line, which also consists of Ormand Beach, Fla., native Brian Kelley, released their first album, ?Here?s to the Good Times,? this month. The album debuted at No. 3 on the Billboard Country Albums chart and No. 10 on the overall Billboard Top 200 chart.

The album also clinched an award, with the highest album sales of 2012 for a new country artist.

Their first single from the album, ?Cruise,? reached the top of the Billboard Country Singles chart on Dec. 15.

The group took part in CMT on Tour this fall, which included Jake Owen and Love and Theft. They will hit the road with Luke Bryan on his first headlining tour, ?Dirt Road Diaries,? in early 2013, although no shows are planned for the local area.

Colt Ford released his fourth studio album in 2012 and as always had help from a wide variety of music business veterans from Jason Aldean, Luke Bryan, Dierks Bentley and even Wayna Morris from the legendary R&B vocal group Boyz II Men.

The album debuted at the top of the Billboard Country charts in August, giving Average Joe Entertainment, a label Ford started with longtime friend and Valdosta native Shannon Houchins, its first No. 1 hit. The album came in at No. 5 on the overall Billboard Albums chart and was not the only hit for Ford this year.

?What?s very unique is Georgia and what?s been going on in Atlanta the last 10, 20 years is you?ve got the rural area and you?ve got a lot of country artists doing their thing, and then in Atlanta you?ve got the whole urban scene exploding,? southern rapper Bubba Sparxxx said. ?It?s just a mixing of flavors and you?ve got me and Colt who mix it all up.?

Another local talent that has tasted stardom this year has worked with Ford a lot in the past.

Jefferson native Brantley Gilbert, who has had a huge following for years while playing the club and bar scene, tasted success in 2011, co-writing the Jason Aldean hit with Ford.

But 2012 has been Gilbert?s breakout year.

Gilbert, who at one time was signed to Average Joe?s Records, signed with Big Machine Records? Valory Music Group.

Valory re-released his album from a previous label, ?Halfway to Heaven,? and Gilbert earned two No. 1 hits with ?Country Must be Country Wide? and ?You Don?t Know Her Like I Do.?

This past summer, Gilbert toured with country superstar Toby Keith on his ?Overdrive? tour. In July, he announced his first headlining tour called the ?Hells On Wheels Tour? with shows including Uncle Kracker on some dates.

It was announced that a second leg of the tour will begin at the end of January, with Tifton native Kip Moore opening for him.

?The coolest thing that?s going on with Georgia is that none of us sound alike,? Ford said. ?We?re all very different, but we all grew up pretty much the same. We have that common thread, but at the same time we all sound different and I think that?s most important.?

Gilbert, Moore and Florida/Georgia Line, will be three of the five artist that have been picked to play at the Country Radio Broadcasters (CBR) for the 2013 New Faces showcase on March 1. They will be playing in front of radio programmers, the people who chose which songs are played on the radio. Previous New Faces performers have been Garth Brooks, Faith Hill and Taylor Swift.

Don't be afraid to stumble at times, it's perfectly normal. Even the biggest online companies out there had a lot of crawling to do before they were able to run. I can assure you that if you take just a few extra steps to keep a positive balance, you will be much more successful and happier with your online business.

Leadership Insight: A Need to return to the Ethic of Excellence.

Isn?t it interesting that we tend to use the words ?exceptional? and ?extraordinary? to describe all-out efforts and quality performance?

In far too many places leaders have allowed the pursuit of adequacy to replace the pursuit of excellence. ?In too many organizations, the culture supports mediocrity and makes it harder to expect and demand best efforts and best performance. What happened to the ethic of excellence?

The focus on compliance rather than values, on doing what is required rather than doing what is right, is just one example of minimalism, but there are others.

Performance reviews are rarely sincere and rigorous. reviewers and HR professionals often seem more concerned about making a record and avoiding conflict or confrontation than in challenging employees to really improve their knowledge and skills. To do better.

Many organizations promoting people because they?ve been waiting in line the longest rather than because they are the best people to do the best job.

In many areas including, police work and medicine, we often see a focus on doing the thing that involves the least legal risk. In my police trainings we use the phrase ?lawful but awful.?

BOTHELL -- A new sports and recreation complex has opened at the University of Washington's Bothell campus.

The $3.3 million complex covers 2.5 acres and is located behind Campus Way NE, next to the North Creek bicycle trail and campus wetland. It includes a shared soccer and softball field, tennis courts, a sand volleyball court and a basketball court.

Construction began in June and a grand opening was held earlier this month. It is scheduled to be open from 8 a.m. to 11 p.m. every day.

The sports and recreation complex was funded by student activity fees and building fees. It's for student use although UW-Bothell staff and faculty as well as students at Cascadia Community College can pay a quarterly fee to use the field, said Betsy Brown, manager of recreation and wellness for the university. The field is not open to the public.

"This is going to offer students an opportunity to interact with each other at a way that they haven't been afforded at this campus before," she said. "I think it's going to increase school pride and help students stay around and want to be on campus more."

Ali Bulzomi was a sophomore at UW-Bothell and a captain of the university's ultimate frisbee team when she and other captains of sports teams started talking about a sports complex for the campus.

She met with Marilyn Cox, vice chancellor for administration and planning, in September 2009 and shared plans that she and other students had put together.

"I had a spiel written out and a drawing of what it would look like and where it could go," said Bulzomi, who now is a budget analyst at UW-Bothell.

Cox liked the idea but said the complex should include a beach volleyball court, Bulzomi said. It was a joke at the time but the finished complex does include a sand volleyball court.

"It started off as a small thing but it got really big," Bulzomi said. "I love it. I think it's beautiful."

Kevin King, 24, is a junior at UW-Bothell who is studying business administration and serves as the UW-Bothell Associated Student Body president. He's used the complex to play basketball, softball and soccer and is looking forward to seeing it used for intramural sports.

"We're very excited to have had the opening this quarter and to have the students be there and play," he said. "I think we got the sizing down just right. It turned out very well."

Even the smallest stroke can damage brain tissue and impair cognitive function

Dec. 16, 2012 ? Blocking a single tiny blood vessel in the brain can harm neural tissue and even alter behavior, a new study from the University of California, San Diego has shown. But these consequences can be mitigated by a drug already in use, suggesting treatment that could slow the progress of dementia associated with cumulative damage to miniscule blood vessels that feed brain cells.

The team reports their results in the Dec. 16 advance online edition of Nature Neuroscience.

"The brain is incredibly dense with vasculature. It was surprising that blocking one small vessel could have a discernable impact on the behavior of a rat," said Andy Y. Shih, lead author of the paper who completed this work as a postdoctoral fellow in physics at UC San Diego. Shih is now an assistant professor at the Medical University of South Carolina.

Working with rats, Shih and colleagues used laser light to clot blood at precise points within small blood vessels that dive from the surface of the brain to penetrate neural tissue. When they looked at the brains up to a week later, they saw tiny holes reminiscent of the widespread damage often seen when the brains of patients with dementia are examined as a part of an autopsy.

These micro-lesions are too small to be detected with conventional MRI scans, which have a resolution of about a millimeter. Nearly two dozen of these small vessels enter the brain from a square millimeter area of the surface of the brain.

"It's controversial whether that sort of damage has consequences, although the tide of evidence has been growing as human diagnostics improve," said David Kleinfeld, professor of physics and neurobiology, who leads the research group.

To see whether such minute damage could change behavior, the scientists trained thirsty rats to leap from one platform to another in the dark to get water.

The rats readily jump if they can reach the second platform with a paw or their snout, or stretch farther to touch it with their whiskers. Many rats can be trained to rely on a single whisker if the others are clipped, but if they can't feel the far platform, they won't budge.

"The whiskers line up in rows and each one is linked to a specific spot in the brain," Shih said. "By training them to use just one whisker, we were able to distill a behavior down to a very small part of the brain."

When Shih blocked single microvessels feeding a column of brain cells that respond to signals from the remaining whisker, the rats still crossed to the far platform when the gap was small. But when it widened beyond the reach of their snouts, they quit.

The FDA-approved drug memantine, prescribed to slow one aspect of memory decline associated with Alzheimer's disease, ameliorated these effects. Rats that received the drug jumped whisker-wide gaps, and their brains showed fewer signs of damage.

"This data shows us, for the first time, that even a tiny stroke can lead to disability," said Patrick D. Lyden, a co-author of the study and chair of the department of neurology at Cedars-Sinai Medical Center in Los Angeles. "I am afraid that tiny strokes in our patients contribute -- over the long term -- to illness such as dementia and Alzheimer's disease," he said, adding that "better tools will be required to tell whether human patients suffer memory effects from the smallest strokes."

"We used powerful tools from biological physics, many developed in Kleinfeld's laboratory at UC San Diego, to link stroke to dementia on the unprecedented small scale of single vessels and cells," Shih said. "At my new position at MUSC, I plan to work on ways to improve the detection of micro-lesions in human patients with MRI. This way clinicians may be able to diagnose and treat dementia earlier."

How To Reduce 401K Fees Through Portfolio Analysis | Financial ...

Do you know how much in mutual fund fees you are paying a year? I didn?t, so I ran my 401K portfolio through Personal Capital?s 401k fee analyzer and I?m absolutely shocked by the results! I always figured that from a percentage point of view, my mutual fund fees were small. But, when you take a small percentage multiplied by a big enough number, the absolute dollar amount starts adding up!

401K Fees Add Up!

As you can see in the picture above, I?ve got four main mutual funds in my 401K account after jumping back into the market post election. I?ve got another fund worth about $22,000 as part of my 401K which does not show a fee, probably because it is a hedge fund whose fees are baked into the performance. Typical hedge fund fees are 2% of assets under management and 20% of upside. This is called 2 and 20, which is egregiously high, but it?s the only way I can get short exposure to hedge my bets.

I?ve been wanting to do a 401K/mutual fund fee analysis for the longest time, but was too lazy to do the analysis until I realized I didn?t have to do the calculations myself. As the new year arrives, I want my portfolio to be as optimized as possible.

ANALYZING YOUR 401K FOR EXCESSIVE FEES

* Huge spread in expense ratios. The cheapest expense ratio is 0.19% for the Vanguard IT Index Fund and a whopping 0.74% for the Fidelity Blue Chip Growth Fund. 0.74% is almost 4X greater than 0.19%, supposedly because they?ve got to pay the fund manager and analysts for providing alpha and outperforming the S&P 500 index. If the fund manager(s) can indeed outperform the S&P 500 index by more than 0.5% a year, then their fee is on par with my cheapest Vanguard Index Fund. If not, I?m wasting my money.

* Fees add up over time. In 20 years, I will have paid ~$87,000 in mutual fund fees if I keep my existing portfolio. I don?t know about you, but that seems a lot, even if my 401k does grow to $1,500,000 as my 401K savings guide estimates. Just doing the math here, 67% or $58,290 of the $87,000 in fees will come from my Fidelity Growth Fund alone. Meanwhile, the Fidelity growth fund only accounts for 39.5% of total assets. Long term growth has a way of compounding into great returns for consistent investors and savers, however it sure does a number on the total amount of fees as well.

* Understanding tax cost ratio.?The Tax Cost Ratio measures how much a fund?s annualized return is reduced by the taxes investors pay on distributions. The range is usually between 0%-5%, the lower the better.?For example, if a fund had a 1.5% tax cost ratio for the three-year time period, it means that on average each year, investors in that fund lost 1.5% of their assets to taxes e.g. a 10% return is really only a 8.5% return. It?s good to see my highest fund with a 0.74% expense ratio have only a 0.11% tax cost ratio! One can simply combine the two to figure out their total expenses. The Fidelity Growth Fund is therefore 0.85% vs. 1.94% for the Vanguard Precious Metals fund! The Vanguard IT Fund is the lowest 0.31%.

* Big fan of index funds. The Vanguard family of funds have some of the lowest, if not the lowest fees in the mutual fund industry. The reason is because of their scale and also because they run passive index funds. There?s no team of analysts to pay. There are no business trips to expense to go kick the tires of the company?s they hold. The index is rebalanced usually once a quarter, or whenever there is a big index addition or subtraction to reduce variance risk. Three out of four of my funds are by Vanguard. After I finish typing this post, all of my funds will be by Vanguard!

* You can create a portfolio of index speciality funds. About 60% of my entire 401K allocation is invested in three speciality index funds: Energy, Metals & Mining, and Technology. If I wanted, I could create a portfolio of 20 speciality index funds if I wanted to. In other words, you don?t have to buy one fund that asset allocates across the industrial board. If you have conviction in particular industries which you think will outperform, there is an index fund for you. We can discuss investment decisions in a future post.

401K PRO FORMA ANALYSIS OF LOST RETIREMENT OVER TIME

This is the chart that really kind of makes me sick. Let?s say I continue to max out my 401K like I have been doing for the past 13 years, get a full employer match (I was getting more due to profit sharing), and assume an annualized return of 5.8%. After 30 years, I will have paid $489,014 in fees and lost out on 2 years worth of retirement income!

The funny thing is, my blended expense ratio is 0.43% compared to Personal Capital?s target benchmark of 0.5%! The public is paying a tremendous amount of mutual fund fees over our lifetimes, and we don?t really know it until we do the analysis. To put it differently, if you want to make big bucks in your life, you should consider a career in money management. It?s all about leverage. A fund manager can run $1 billion dollars under management as easily as he can run $10 billion dollars under management.

401K Lost Performance

CONCLUSION

It?s important to not only look at the expense ratio, but also the tax cost ratio to see how much your performance is getting reduced by each year. Look to invest in lower cost index funds instead of actively managed funds who have shown a poor track record of performance. There are plenty of studies showing that the large majority of actively managed funds underperform their benchmarks.

Personal Capital helped me realize that I am paying at least $1,000 more a year in mutual fund fees than I should by investing in the Fidelity growth fund. I wouldn?t have been able to see what an outlier the fee is without the 401K fee analyzer that aggregates all the data and provides a side by side comparison. Not bad for a free online financial management huh??I suggest everybody take a hard look at their finances to see where they are paying unnecessary fees.

To bring up the second chart in the post, click the ?401K Fee Analyzer? link under the Investment tab and play around with the assumptions. It?s best to use three assumptions: conservative, base case, and blue sky to have a feeling of where your 401K will be when you want to retire. I have a feeling you are going to be as surprised as I was about how much you are paying in fees. After writing this post, I?m exchanging my Fidelity Fund into a Vanguard Large Cap fund.
